Fixed teeth on implants

What All-on-X actually means.

A full arch of teeth screwed onto your own implants. Nothing comes out at night, nothing covers the roof of your mouth, and nothing needs adhesive. All-on-X simply means we use the number of implants your bone actually calls for – four, six, or more.

What All-on-4 means

All-on-4

A full arch carried on four implants, angled to use the bone you still have. Fewer surgical sites and a shorter appointment – the right choice when bone volume and bite force allow it.

What All-on-6 means

All-on-6 / All-on-X

Our flagship: six or more implants spreading the load across the whole jaw. Maximum stability, the strongest bite, and better long-term protection against bone loss – which is why most of our full-arch patients end up here.

Full arch of fixed teeth carried on titanium implants
  • Comfortable
Local anaesthesia, with sedation available for anxious patients.
  • Never moves
Screw-retained onto the implants – it does not lift, click or slip.
  • Eat what you want
Steak, apples, corn – a bite strength close to natural teeth.
How an implant is built

Three parts, one natural tooth.

Understanding the parts makes every quote easier to read – and makes it obvious what a cheaper offer has left out.

Crown, abutment and titanium implant
1

The crown

The visible tooth, matched to the shade and shape of your face.

2

The abutment

The connector that joins the crown to the implant below the gum.

3

The implant

A titanium root that fuses to your jawbone and carries the bite.

Is this your case?

These are the situations we see every week. However long it has been, and however bad you think it looks, a full-arch plan usually still works.

Dental travel to Costa Rica

Your trip, start to finish.

Patients fly in from across the USA and Canada. Nothing about the trip is left for you to figure out alone – your coordinator plans it with you before you book a flight.

  1. Step 1

    Send your case

Photos of your smile, and a panoramic X-ray if you have one. WhatsApp is easiest – someone answers around the clock.

  2. Step 2

    Virtual consultation

A video call with the clinic to go through what is possible, what it involves, and how many days in Costa Rica your case really needs.

  3. Step 3

    Written plan & dates

You receive the treatment plan and the full price in writing, plus the hotel booking and your day-by-day schedule. Then you book the flight.

  4. Step 4

    Treatment in San José

3D scan on arrival, surgery, and fixed provisional teeth. You rest at the partner hotel a short walk away and come back for post-op reviews.

  5. Step 5

    Home & follow-up

You fly home with fixed teeth. We check in remotely while the implants integrate, and plan the short second visit for your final prosthesis.

Where you stay

A short walk from the chair to your bed.

We work with a partner hotel within walking distance of the clinic, so the day of your surgery does not end with a taxi ride across a city you do not know. Your coordinator arranges the booking with your treatment dates – you just confirm it.

  • Walking distance Partner hotel steps from the clinic in Sabana Sur
  • 200 Rooms available to our patients
  • Arranged Booked around your treatment schedule
  • Companion Travelling with someone is welcome – tell us and we plan for two

Dr. Erick Rivera

Founder & director · BISHO Facial & Dental Center

Dr. Rivera has practised dentistry in San José for 15 years and built Bisho around implant and reconstructive work – the cases other clinics call complicated. Full-arch rehabilitation with All-on-6 and All-on-X is what he does most, and what he plans personally for every patient who travels to us.

He treats patients in Spanish and English, and prefers to say plainly what a case needs – including when the honest answer is that a simpler treatment is enough.

Frequently asked questions

Your questions, answered.

How many days do I need to be in Costa Rica?

It depends entirely on your case, which is exactly why we review your scan before you book anything. Once Dr. Rivera has seen it, you get the number of days in writing along with your plan – so you can book flights and time off with confidence instead of guessing.

Will I fly home with teeth, or without?

With teeth. In most full-arch cases we place a fixed provisional prosthesis during your stay, so you leave with teeth that are screwed in place – not something you take out at night. The definitive prosthesis is fitted once the implants have integrated with the bone, usually after a few months.

I was told I do not have enough bone for implants.

We hear this constantly, and it is often out of date. A 3D CBCT scan shows the bone you actually have, and full-arch techniques are designed to use it – angled implants, longer implants, or grafting where it is genuinely needed. Send us your scan and you will get a straight answer either way.

Does it hurt?

The surgery itself is done under local anaesthesia and you should not feel pain during it; sedation is available if the idea of the appointment worries you. Afterwards expect some swelling and soreness for a few days, managed with medication – most patients describe it as far less than they had braced themselves for.

Can I be treated if I have diabetes or high blood pressure?

In most cases yes, provided the condition is controlled. Tell your coordinator about your medical history and any medication – particularly blood thinners or bone medication – at the virtual consultation, so Dr. Rivera can adapt the plan before you travel rather than on the day.

Is the whole team comfortable in English?

Yes. Dr. Rivera treats in English and Spanish, and our patient coordinators work in both languages – from your first WhatsApp message through to your follow-ups after you are home. You will never be handed a consent form you cannot read.

What happens if something needs adjusting once I am home?

You keep the same WhatsApp thread you started with. Send a photo, and we will tell you whether it is normal healing, something your local dentist can handle, or something we should see – and we plan the second visit for your final prosthesis around your calendar.
